1. Acceptance of Terms
By accessing or using the website at adhdanxietydepressionhelp.org (the "Site"), operated by Dynamic Mental Health Services LLC ("we," "us," or "our"), you agree to be bound by these Terms of Service ("Terms"). If you do not agree to these Terms, you must not use this website.
These Terms apply to all visitors, users, patients, and others who access or use the Site. We reserve the right to update or modify these Terms at any time, and your continued use of the Site following any changes constitutes your acceptance of the revised Terms.
2. About Our Services
Dynamic Mental Health Services LLC is a telehealth psychiatric practice operated by Zinny Ezete, APRN, PMHNP-BC, a board-certified Psychiatric Mental Health Nurse Practitioner. We provide comprehensive psychiatric services including:
- ADHD evaluations and management
- Anxiety disorder treatment
- Depression treatment
- Medication management
- Psychiatric evaluations for adults and adolescents ages 13 to 17
Our services are provided exclusively via telehealth and are currently available only to individuals physically located in New Jersey, Connecticut, or New York at the time of their appointment. We are unable to provide services to individuals located outside of these states due to licensing requirements.
Dynamic Mental Health Services LLC is not an emergency or crisis service. If you are experiencing a psychiatric emergency, please see Section 9 of these Terms.
3. Not Medical Advice
The content on this website, including but not limited to text, graphics, images, and other materials, is provided for general informational and educational purposes only. Nothing on this Site is intended to be a substitute for professional medical advice, diagnosis, or treatment.
You should never disregard professional medical advice or delay seeking treatment because of something you have read on this website. Browsing this website, submitting a contact form, or scheduling an appointment does not establish a provider-patient relationship. A provider-patient relationship is established only after a formal clinical intake has been completed and mutually agreed upon by both the patient and the provider.
Always seek the advice of your physician or other qualified health provider with any questions you may have regarding a medical condition.
4. Appointment Scheduling
Appointments may be scheduled through our Calendly booking page or through our patient portal. By scheduling an appointment, you agree to the following:
- Cancellation policy: We require a minimum of 24 hours' notice for appointment cancellations or rescheduling. Failure to provide adequate notice may result in a cancellation fee or forfeiture of your appointment slot.
- No-show policy: Patients who fail to appear for a scheduled appointment without prior notice ("no-shows") may be charged a no-show fee and may be subject to restrictions on future scheduling.
- Accurate information: You agree to provide accurate, current, and complete information when scheduling appointments, including your legal name, date of birth, contact information, and insurance details (if applicable).
- Eligibility: By scheduling an appointment, you confirm that you are physically located in New Jersey, Connecticut, or New York and are eligible to receive telehealth services in the state where you are located.
5. Telehealth Consent
Our practice delivers care exclusively through telehealth (audio-visual communication technology). By engaging in our services, you acknowledge and agree to the following:
- Telehealth services are conducted through secure, HIPAA-compliant video platforms with end-to-end encryption.
- You consent to receiving psychiatric care via telehealth as an alternative to an in-person visit.
- You understand that telehealth has potential benefits (including convenience and reduced travel) and potential limitations (including technology-dependent service delivery and the inability to perform physical examinations).
- You are responsible for ensuring a private, quiet environment during your telehealth session with a stable internet connection.
- You understand that in certain circumstances, the provider may determine that telehealth is not appropriate for your care and may recommend in-person evaluation or treatment with another provider.
- A formal telehealth consent form will be provided during the intake process and must be signed before services can begin.
6. Insurance and Payment
Dynamic Mental Health Services LLC accepts most major insurance plans through Headway, our insurance verification and billing partner. By using our services, you agree to the following payment terms:
- Insurance verification: We will verify your insurance benefits prior to your appointment through Headway. However, verification of benefits is not a guarantee of coverage or payment. You are responsible for understanding your own insurance plan, including any referral or prior authorization requirements.
- Patient financial responsibility: You are responsible for all copayments, coinsurance, deductibles, and any services not covered by your insurance plan. Payment is due at the time of service.
- Private pay: For patients without insurance or who prefer not to use insurance, private pay (self-pay) options are available. Current self-pay rates will be provided upon request.
- Outstanding balances: Unpaid balances may result in suspension of services until the account is brought current. We reserve the right to use lawful collection methods for overdue accounts.
7. Intellectual Property
All content on this website, including but not limited to text, graphics, logos, images, page layouts, and the overall design and compilation of the Site, is the property of Dynamic Mental Health Services LLC and is protected by United States and international copyright, trademark, and other intellectual property laws.
You may not reproduce, distribute, modify, create derivative works from, publicly display, publicly perform, republish, download, store, or transmit any materials on this Site without our prior written consent, except for:
- Temporary storage in your web browser cache for personal, non-commercial viewing purposes
- Printing a reasonable number of pages for personal, non-commercial use
Any unauthorized use of the materials on this Site may violate copyright, trademark, and other applicable laws and could result in legal action.
8. Limitation of Liability
To the fullest extent permitted by applicable law, Dynamic Mental Health Services LLC, its owner, employees, agents, and affiliates shall not be liable for any indirect, incidental, special, consequential, or punitive damages, including but not limited to loss of profits, data, use, goodwill, or other intangible losses, arising out of or related to:
- Your access to or use of (or inability to access or use) the Site
- Any conduct or content of any third party on the Site
- Any content obtained from the Site

- Technical failures, interruptions, or errors in the operation of the Site
This limitation of liability applies regardless of the legal theory on which the claim is based, whether we have been advised of the possibility of such damages, and whether the damages were foreseeable. This limitation does not apply to liability arising from gross negligence, willful misconduct, or violations of applicable law that cannot be waived by agreement.
Nothing in these Terms is intended to limit or exclude liability for professional malpractice during the provision of clinical services, which is governed by applicable state law and professional standards of care.
9. Emergency Disclaimer
Dynamic Mental Health Services LLC is NOT an emergency or crisis service.
If you are experiencing a medical emergency, psychiatric crisis, or are in danger of harming yourself or others, do not use this website or wait for a response from our office. Instead, take one of the following actions immediately:

Our telehealth practice is designed for scheduled outpatient psychiatric care and is not equipped to handle emergencies. Response times for messages, emails, and appointment requests are not immediate and may take up to several business days. In a crisis situation, always seek immediate help through emergency services.
10. Governing Law
These Terms of Service and any disputes arising out of or related to your use of this Site shall be governed by and construed in accordance with the laws of the State of New Jersey, without regard to its conflict of law principles.
Any legal action or proceeding arising under these Terms shall be brought exclusively in the state or federal courts located in the State of New Jersey, and you consent to the personal jurisdiction and venue of such courts.
11. Changes to Terms
We reserve the right to modify or replace these Terms of Service at any time at our sole discretion. When we make material changes, we will update the "Effective Date" at the top of this page.
It is your responsibility to review these Terms periodically. Your continued use of the Site following the posting of any changes to these Terms constitutes acceptance of those changes. If you do not agree to the revised Terms, you must stop using the Site.
